Can you just discard a card from your hand or is it like sacrificing where there has to be a reason for it?

There has to be a reason for it, otherwise, Madness would be CRAZY. Note that, if you end YOUR turn with more than 7 cards in hand, you have to discard until you get to 7, but if you get more than 7 cards in hand during your opponent’s turn (Instant card-drawer, for instance, like Think Twice), you’ll only have to discard at the end of YOUR turn anyway.
