I have three starter decks from Kamigawa. Let's say I just used cards from that series (Champions of Kamigawa, Betrayers of Kamigawa, Saviors of Kamigawa). Would that be standard?

What cards, if any, are banned?

Standard has no banned cards. Standard is always composed of the last Core Set (from M11 onward, when it comes out, the last TWO Core Sets, although that might change) and the last two blocks. So, as of now, Standard is comprised of M10, Shards of Alara Block (Shards of Alara, Conflux, Alara Reborn) and Zendikar block (right now, only Zendikar). Extended is comprised from 8th edition onwards (as far as Core Sets are concerned) and Onslaught block onward, so Kamigawa cards would be Extended legal. And as for Extended, I reckon Aether Vial and Sensei's Divining Top are both banned, but I don't know if anything else.
