The Shire Countryside and many other cards. Each time in these sort of instances if you have 4 of those cards out can you heal 4 companions?

Rout , same thing there. exert 2 companions 4 times if you have 4 out??!

Or does each time only happen once and can only use 1 card?

It happens once for each copy of the card in play, so yes, 4x Rout would force 8 exertions every time the FREEPs move without initiative, and 4x Shire countryside would allow 4 heals for each burden removed.
