If Frodo is killed, does Sam get a burden?

I think so...but that's a fantastic question.
I don't think that Sam becoming the Ring-bearer should've ever been a response...rather:
If Frodo is killed, make Sam the Ring-Bearer (resistance 5).
I can't really tell if that because Sam's action is a response it comes after the "each time" action of Slag Mounds.

Hmm I would suppose that the burden would be added first because the Slag mounds action is a required action. The Response action on Sam is optional. Story-wise it really would make sense that Sam would have to add it.

That's true. The Free People's player doesn't have to make Sam the ring-bearer. If they want to lose.

How could you add the burden to Frodo after he was already killed?

I think that, in this situation, Slag Mounds will have no effect. If I remember correctly, the the timing goes like this:
1. Frodo is killed
2. Slag Mounds (as well as any other mandatory response) triggers; as there is not currently a Ring-bearer, the effect fizzles (nowhere to put the burden).
3. The Free Peoples player can use Sam's optional response to make him the new ring-bearer.

Thought it goes that a companion isn't placed in the dead pile when killed until all actions responding to the kill have resolved (so cards like Theoden TaP can trigger, especially if there's no "stack")... though I'm fuzzy on what if anything can still affect the killed companion--heck, if it's still considered to be in play.
Anyone remember the details?

I think the burden fizzles like RedGold said (perfect verb for it, too!)

No, he's absolutely murdered.
I'd play it so that the burden fizzles, too. Maybe the card could be thought of as being in limbo (similar to an event) so it's not affected by anything requiring it to be in play, even though it can still affect the game. Then it hits the discard pile. But I don't really have anything to back that up.

Burden fizzling makes the most sense to me anyway... isn't the "each time" a mandatory response, vs. the optional Sam response? And mandatory comes before optional, right?

Right, though I think what makes Slag Mounds mandatory is the lack of "may".
