Title: Southron Savage

How exactly does his text take effect? Are minions and twilight counted separately?

While the total number of minions and twilight tokens is 3 or fewer, Southron Savage is str +3 and fierce.

Is it enough if there are 2 minions and 2 twilight tokens for his bonus not to work or must there be either 4 other minions or 4 twilight tokens?

They are totaled together.   Like this:

If (number of other minions) + (number of twilight tokens) < 3, then he is strength plus 3 and fierce.

My recommended solution.

Don't play him.
