New warning system

I've added a forum feature to allow moderators to deal with warnings for users that violate forum rules. For the most part, a warning will be applied when the mods have discussed the situation and decide it is necessary. If you are warned, a note will be added to your post with the reason and you will receive a PM.

Levels of warnings are displayed next to each username.
1: Warned (defaults to 3 days)
2: Moderated (all posts must be approved - defaults to 7 days)
3: Muted (no posting - defaults to 7 days)
4: Banned

The number of days can be chosen per scenario if necessary. It is VERY unlikely that a user will be banned, but if it had to happen, it would be obvious to the entire forum as to why. Only the admins will handle banning.
