Khamûl seems to be the only one named by Tolkien, corresponding to Ulaire Attea, The Easterling (article on the names here).

Nope, they were never named except for Khamul. Though MERP and MECCG did a nice job at inventing names for them, like Er-Murazor (the witch-king), Akhorahil, Adûnaphel and Indur.

Decipher had an article on the naming convention early in the Fellowship days.  It basically boiled down to: creating a name would require approval from the franchise, and borrowing a name could have trademark problems, so they settled by naming all non-Witch King Nazguls as "Nazgul One" "Nazgul Two" etc.  Afterwards they converted them to Elvish so they didn't sound so silly.
